The Home Care Nurse is responsible for providing nursing care to patients in their homes. This may include administering medication, monitoring vital signs, providing wound care, and educating patients and their families on managing their conditions.

Responsibilities:

• Develop and implement nursing care plans
• Administer medications and treatments as prescribed
• Monitor patients' vital signs
• Provide wound care and other treatments as needed
• Assess patients' conditions and report any changes to the healthcare team
• Educate patients and their families on managing their conditions and promoting health
• Collaborate with other healthcare professionals to provide comprehensive care
• Maintain accurate and up-to-date patient records
